CenterWell Senior Primary Care - - Responsibilities: Provide direct patient care to patients under direction of the RN and according to the Aide Plan of Care; Assist with self-administered medications and monitor medication usage; Take and report accurate vital signs (temperature, pulse, blood pressure); Assist patients with personal hygiene including bathing, grooming, oral care, and skin care; Assist with feeding, nutrition observation, and safe meal positioning/feeding; Assist with toileting, transfers, mobility, and ambulation per Plan of Care; Document interactions and care provided to ensure continuity of care
