CenterWell Senior Primary Care - - Responsibilities: Provide direct patient care to patients under the direction of the RN and according to the Aide Plan of Care; Monitor and report abnormal vital signs (temperature, pulse, blood pressure) to supervisor; Assist with activities of daily living including bathing, grooming, feeding, toileting, mobility; Document interactions with patients, caregivers, doctors and staff for continuity of care; Maintain infection control practices and a clean, safe environment
