Arizona License
Valid fingerprint clearance card
REPORTS TO: Principal
WORKS CLOSELY WITH: Principals and other support staff.
JOB GOAL: Provide students with medical care as needed in a timely manner. Keep the health office organized and records maintained on a daily basis.
PERFORMANCE RESPONSIBILITIES:
Evaluate and maintain a current health record, including a health and developmental history, and an immunization history.
Systematically record data on the student's cumulative school health record and maintain a charting system of student visits and treatment provided in the nurse's office.
Verify immunization records are current.
Screen and evaluate students for deficits in vision, hearing, growth, and blood pressure, when appropriate and will refer if necessary.
Provide necessary health counseling and refers to an appropriate professional when indicated.
Inform administration of communicable health problems.
Intervene appropriately for individuals and populations at risk for preventable, potential health problems.
Complete school, county and state reports in a timely manner.
Use community resources for referral of students with unmet health needs in the school.
Collaborate with agencies to ensure continuity of service and care.
Participate as a member of the assessment and placement committee for special education programs whenever required.
Serve as a liaison among the parent, school, and community in health matters.
Communicate effectively and appropriately with students, teachers, and parents.
Engage in continuing education and professional activities to facilitate maintenance of quality health and nursing services to students.
Apply appropriate theory as basis for decision making in nursing practices.
Respond in a timely manner to emergency situations.
Perform other tasks and assumes other responsibilities as Paradise Schools may from time to time assign.
