HomeWell Care Services - - Responsibilities: Follow Care Plans: Provide care in accordance with individualized client care plans; Comprehensive Support: Assist with companionship, meal preparation, light housekeeping, shopping, transportation, medication reminders, and personal care; Communication: Promptly report any changes in client condition, concerns, or emergencies to the Care Manager; Safety Compliance: Adhere to infection control standards and utilize PPE as required; Timekeeping: Accurately clock in/out and document completed tasks using ClearCare Telephony
