CPLC LUCES provides HIV prevention education and supportive services to people living with or at risk of HIV/AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.
Mental Health & Psychosocial Support Services: Individual or group behavioral health interventions include Healthy Relationships, CLEAR, and Sin Buscar Excusas. Mental health and substance abuse counseling services including Spanish-language support groups Mujeres Poderosas, Manos de Esperanza, and/or individual counseling sessions online or in-person.
Ending the HIV Epidemic (EHE) Initiatives: "Fuerza Positiva University" is a Spanish and English, 12 session curriculum tailored towards Latinx newly diagnosed individuals that are seeking to take control of their HIV diagnoses through education and empowerment. Online and in-person sessions.
POSITION SUMMARY Under general supervision, provides direct covered services and treatment to individual clients, coordinates with community support and stakeholders. Responsible for managing appointment schedule as well as conducting targeted outreach to people with a known HIV disease so they may be enrolled in care and treatment services.
Psychosocial Specialists have a variety of responsibilities, including, but not limited to:
Facilitating weekly dynamic EHE Initiatives Fuerza Positiva University peer support psychosocial groups for Latinx individuals living with an HIV/AIDS diagnosis in Maricopa County in Spanish
Educating group members on the fundamentals of the HIV virus and its effects on the body
Discussing in-depth the HIV life cycle
Discussing how outside oppressors and barriers, such as, religion, stigma, HIV diagnosis disclosure, and family dynamic affect the day-to-day life of Latinx individuals living with HIV
Empowering clients with skills connected with activities of daily living
Providing non-judgmental, unconditional support to clients
Implementing of evidence-based CDC behavioral health interventions: Healthy Relationships, Choosing Life; Empowerment, Action, Results (CLEAR), Sin Buscar Excusas/No Excuses
Collaborating with Arizona Department of Health and Maricopa County to facilitate state and local initiatives
Supporting client from start to finish of program, including referrals to community resources, client graduation from Fuerza Positiva program, and transition to ongoing psychosocial support services
Developing and distributing outreach materials, directly contacting targeted community members, networking with appropriate community organizations
JOB DEMANDS Ability to travel within Maricopa and Pinal counties as needed
MINIMUM QUALIFICATIONS AND COMPETENCIES
Must meet one of the following educational/experiences:
Has an associate degree from an accredited college or university in a field related to case management such as social work, human development, nursing, public health or other human services related field preferred
Or high school diploma with two years of experience in the field of HIV, or a high school equivalency diploma with two years of experience in the field of HIV
Knowledge of HIV/AIDS and other STD transmission
Must have access to safe transportation, valid Arizona driver's license and automobile insurance as required in the State of Arizona. Must be willing to travel throughout Maricopa County
Bilingual language skills in English/Spanish required
Must be atleast 21 years of age
Other Competencies
Able to work a flexible schedule as needed per program requirements
Have good organization and time management skills
Must have basic computer skills; possess good written and verbal communication skills
Knowledge of the Ryan White healthcare system, a plus
Must be capable of working with diverse cultural population
Knowledge of community resources and ability to assist clients in securing such services
